I have just brought my 8 week old baby home from NICU and would like to start her on the EASY routine. She was 6 weeks and 1 day early, I would like to know whether I should be looking at sample routines of her current age or her adjusted age?

Congratulations on your LO, I am sure it's a delight to bring her home!
You should be looking at her adjusted age, so 2 weeks old. Here are the samples:
chronological EASY samples, 0-3 months
and good luck!

So I guess that means I should change her to the 4/4 routine 6 weeks later than usual too, is that correct?

Do you mean the 4h EASY? Usually one starts gradually increasing the A a couple of weeks before 4m old, depending on naps, so you would start and transition when her adjusted age is 3.5m approx.
