Title: 4 Month old - how do I get him to sleep through the night???

I have a 4 month old boy, and he has not slept through the night yet... and now he is waking every hour. How do I train him to sleep through the night?

Schedule:
E - 5:30a
S- 6:00-7:30

E- 8:00
A-
S- 9:30-11:30

E- 12:00
A-
S- 1:30-3:00

E- 3:30
A-
S- 4:30-5:30 (catnap)

E- 6:00
A-
S- 7:00- 8:00 - (catnap)

E- 8:30
A-
S- 8:30-9:00

Wake 11:00 - I usually wait to see what type of cry...when he really starts going, I go in to his room try to swaddle him, pacify, and then shush pat - sometimes rock if needed
Wake 12:45am - swaddle, pacify, shush pat
E-1:00am - feed
Wake 2:00am - swaddle, pacify, shush pat
Wake 3:00am - swaddle, pacify, shush pat
Wake 4:30am - swaddle, pacify, shush pat

E- 5:30am -feed

do you think maybe the paci has become a problem? If he's waking every hour and needing the paci to go off to sleep, my first guess would be to get rid of the paci.
