Title: Please help a v.confused mum - Hungry or Habitual Waking??

I'd be very grateful if someone could offer some advice on whether they think my DD is waking at night from hunger or just from habit.
She's breast fed in the day, then has formula at night, she's 13 weeks old and weighs 12lbs 7oz. When she was 9-10 weeks old we had a fortnight when she slept from 9.30pm (ish) to 4am (ish) and we thought we were on the right track but her pattern has since changed :(
She now wakes at 1.30-2 every night and then again at 5-6. We're currently trying to adopt the E.A.S.Y routine and have been topping up her feeds in the day to make sure she's getting enough calories and have introduced a dream feed but still no change. When she does wake at night she will always feed and normally takes 150ml but I don't know whether she's still just used to being fed when she wakes up.
Don't want to deny her food but equally don't want to encourage habitual waking. Please help!!

There is a major growth spurt at this age and I am inclined to think hunger.  But can you post your routine in EAS format?

Definitely hunger.
