Could you re-post her Wake and Sleep times?

My gut is saying get rid of that pacifier....but I'll reserve that until I take a look at the wake and sleep times. ;)

If she wakes for a few hours, does it always start at either 1 or 3?

Sure:

7:15 - Wake Up (hooray) and bottle (6-8 oz)
8:15 - Breakfast
9:30-11:00 - Nap
11:00 - Bottle (6-8 oz)
12:00 - Lunch
1:30 - 3:00 - Nap
3:00 - Bottle (6-8 oz)
6:00 - Dinner
7:00 - Bottle (4 oz)
7:30 - Bathtime
7:50 - Bottle (4 oz) and bedtime.

As far as the wakeups -- they seem to be happening around 12-1 and around 3-4...(which, ironically, is when she used to wake up for feedings before she started sleeping through the night)

Have you tried W2S? Those wakes might be habitual especially if they're happening at the same time, and those times correspond to old feed times. You might try that. I'd start with the first one of the night - so go in and kind of jiggle her a little (but not too much so she fully wakes) at about 11:30 and see what happens.

Also, you might want to consider showing her where the paci is rather than handing it to her. If you've got a bunch in there and keep them in a little pile she should be able to find them on her own. That might keep you from having to go in.

Schedule looks good. I was going to say you might want to consider shortening that afternoon nap a bit and scooting bed time back, but I don't want you to loose that late morning, so you might want try the W2S and paci thing first........
