Bottle Feedsing Morning/Night at 2 years old - Help

Yes we created the problem and know has come time to fix it.  As two working parents with lots of feeding problems with our DS including dysphasia we have always really enjoyed the peaceful time of holding him at night and in the morning and giving him a bottle of milk (2% now). The problem is that we think we have waited too long to remove it now as he demands it first thing when he wakes up and he has started waking up wet probable from the last bottle at night. 

How do we go about breaking this habit? 

Any personal stories would be great to confirm we are not the only parents that have done this. :(
