I'm reasonably new to the board and new to being a mum!

My (spirited/touchy) son is now 15 weeks old and we have been following a (very) rough EASY routine for most of this - mostly E 3, then 4 hourly. The wee guy has been suffering from reflux, so a typical routine has been near impossible! We seem to have this under control now, so looking to get a better routine in place.
He is on a 4 hourly schedule, but i'm not sure how much A time he should be having?
Unfortunately he takes after his daddy and is an early riser! Usually waking at 5.30 but doesn't cry to be fed and chats to himself for a while. We then usually feed at 6.30-7. How does the A time work with being awake an hour before being fed?
We've also rocked him to sleep for most of the time, so trying without much luck to get him off to sleep on his own. Would really appreciate a good technique for this... as much detail as possible - it's going to be hard going!
Napping has always been a problem, and he gets OT and irritable very quick, leading to short naps and a vicious circle! Getting the 45min naps just now. I've tried HTTJ but he wakes up screaming regardless... is it something to persevere with?
Finally...! I can't be at home all day to get naps in his crib, how to I encourage him to sleep when we are out? Walking in the pram used to be fail-safe but isn't anymore and I think I have the only baby in the planet who doesn't sleep in the car!!!
I know he is OT alot which makes him really (really) grumpy, but it seems no matter when I try to put him to sleep it is a battle.
