19 week old waking every 2-3hrs!

Hi everyone

I'm new to this site and was hoping for some advice.
My 19 week old son has never been a great sleeper. He used to go 4-6 hours a stretch which was ok but lately hes been waking every 2-3 hrs!
I've never done a dream feed as he naturally wakes around 11-12pm (bottle feed 6oz). Waking times after this are erratic and takes long time to resettle lots of ssshh/pat.  If he does not resettle i usually give breast as i have been trying to keep this up, they don't feel that full as mainly bottle feeding but can't believe i would have to bottle feed again when he wakes 
Could be teething keeping him awake, calpol dosent seem to work!

Also has eczema on crook of arm using Oilatum (dosen't work that well!)
Another thing lately hes been waking up and when i go to him hes rolled over onto his tummy! should i swaddle him, is he too old for this?  He refuses paci and has not found his fingers.

Hes only gone through the night once  :'(

routine:
6.30am - awake (turn mobile on whilst getting daughter ready for school)
7.10am - E
7.30 - A
9.00 - S
11.00- E
11.30 - A
1.00pm - S
3.00pm - E
3.30 - A (on school run awake/asleep)
6.15pm -Bath
6.30pm - Bottle/lullaby
7.00 pm - Bed

Hi there, just to confirm your LO is just over 4 mths now?

At 4+ mths we would normally see him still taking 3 naps and only doing A times of up to 2.25 hrs. Your LO seems to be on high A times, but if his naps are 2x 2 hrs then it might be ok for him.  However, his final A time is really long.  Could you think about capping his second nap at 1.5 hrs so you can squeeze in a CN to stop him getting overtired by bedtime?  This can often contribute to NW.  I'm just thinking if you had him up at 2:30 and then did your school run at 3:30 you can have him down again at 5 for a very short kip.  Then bed would be pushed out just a tad but he would be better rested going down for it.

It could be teething, can you try ibuprofen instead of Calpol? 

Rolling is a very developmental thing.  Lots of practice during the day often helps with that one.

Will try a catnap at 5 then try slightly later bed time.

He is now 23 weeks old

With regards to NW as i've never done DF should i start now at say 10pm and see if this works.  If he still is waking at 3/4am should i feed again? can't believe he would need feeding and suspect its habit, is it best to offer water?

He is having 7oz per feed x 5 bottles per day so don't think problem with calories.  He is slightly under weight which is why upped amount but have not seen improvement after 3 weeks.
