6 month old refusing bottle

Hello, I have a 6 month old girl who has refused to take a bottle from around 3 months old. She still feeds every 2 hours during the day and I am now desperate for some way to get her to take a bottle as I prepare to go back to work and would love some advise.
I have tried every bottle and teat (tt, avert, nuk, breast flow) with a range of different milk (bm, aptimal, hipp, sma, cow and gate) with different people feeding her (myself, dad, grandma) to no avail. I believe that to this point I haven't been strong enough to fully commit to it, but now I am and want to tackle this in earnest.
I have a couple of specific questions;
Is it better to try with breast milk as I know she likes the taste rather than trying to introduce bottles and formula milk?
Is it better that I do the feeding as she associates me with food?
Do you recommend going cold turkey? Once you have made the decision don't offer the breast again or is a gradual approach the best?
Is there a best time of day to start?
Is it possible that she won't ever take a bottle?

These two threads are chock full of advice and support for switching to a bottle and the final outcome was that LO has started taking a bottle happily:
