12 week cat nap issues

Hi all,
      I am new to this Forum and have gone through many posts about cat napping. I have my concerns and i really hope someone helps me out here.
      I have a 12 week old daughter who sleeps through the night anywhere from 10-11pm till 6-7Am. after this sleep she feeds and goes back to sleep by 8 or 8:30 AM and sleeps for another 2 hrs. Once this is done she is awake the whole day till around 8pm. She looks extremely tired throughout the day and is not much active or smiling. It kills me to see her like this.
     I clearly know she needs sleep. so after 11Am everytime she is awake for 1 hr or 90 mins. i try to pick her up at the first signs of tiredness and put her to sleep. she just naps for 15-20 mins.. If i put her in the crib to sleep she would wake up in just 10-15 mins crying unable to settle back to sleep. I have no idea what to do. I have tried EASY method and i have failed many time. she would not want to feed sometimes when she wakes up and we would end up feeding around her sleep time. So this might be affecting her food intake too. I am too confused. All i want is my baby to be able to nap well during the day. Please help.

    Here is her activity log from past 3 days.
    7Am - Eat
    7:30 Am - Activity
    8:30 Am - sleep
   
    11:30 AM - Eat
    12 PM - Activity
    1:40 PM - sleep 20 (mins)

    2PM - 7:30 PM she would take another 15-20 min nap.
    8PM - Bath immediately goes to sleep after that
    8:30 - 9PM - Sleep

    After 9 she feeds and finally goes to sleep between 10 and 11pm.

    I havent put up her feeding time it would normally be every 2 and a half to 3 hrs. The issue i have is sleep.
    Please help me figure how i can make my baby happy and well rested.

Hi and welcome to the forums! :)

How lovely to be getting such good sleep at night, but I know that is of little consolation when your baby doesn't sleep much during the day.  Do you think she is in discomfort?  Often those short 10-20min naps can indicate discomfort.  Does she have any signs of reflux? Reflux 101 - General reflux information

At 12 weeks, her A time is likely around 1.5hr, give or take and it does look like that's what is working in the morning.  We typically advise to work off of a 12hr day/night routine.  So, you might try something like this...

Wake/Eat 7am
A
S 8:30-10:00am
E 10:15am
A
S 11:30-1pm
E 1:15pm
A
S 2:30-4pm
E 4:15pm
A
S 5:30-6:15pm
A wind down activities for BT
E 6:45/7pm
BT 7:15ishpm
DF 10:30pm

Do you have a good wind down that you follow for nap times? What does a good wind down consist of (Includes 4S ritual)  Are you attempting to get her to sleep/resettle using shh/pat?
