I kept up the four feeds until about 10 months or so, and ds gave up the midmorning feed when he started dropping to one nap. I think most pediatricians would suggest substituting something for the nursing, either a bottle of ebm (which could later be a cup of milk) and/or a small snack.
At 12 months, Grace's schedule was
8:00 wake and breakfast
10:00 small snack
10:15-11:45 nap
12:00 lunch
2:00-3:30 nap
3:30 small snack
5:30 dinner
7:30 sleep
Some kids like a cup of milk or small snack before bed too. Grace weaned herself right at a year, I just offered snacks instead of the breast and she didn't seem to mind at all. I also gave her soy milk for each snack/meal per her doctor because she was milk intolerant.
At 13.5 months, Max is still nursing three times per day, but would happily take a cup of ebm if I weren't so lazy! I just say "come on nurse, so I don't have to wash the pump!"  Although - I'm ok with his still nursing past a year.
