Almost 4 months and troubles starting... Lots of Questions!!!

Help!
My 15 week old son was doing very well with EASY up until about a week ago.  I think he may have been in a growth spurt because he wanted to eat all the time.  He had always had a hard time naping over 45 minutes, but the past few days I cannot get most his naps over 30 min!!  I did wake to sleep today and it did not work.  He wakes up smiling and fully awake.  Not sure if I should try to get him back to sleep, it would take forever.  So tonight I have a super overtired baby who took 2 hours to get to sleep.  He is usually in bed by 7:00.  I am going to try to change him to the 4 hour easy, but he has just been sleeping so poorly I am not sure what to do! 

I also have a question on getting him to sleep.  I have been following the book and shushing and staying with him until he is asleep.  For the past 2 days he has started to wake after 5 or 10 minutes again too.   I have tried leaving the room, but he will eventually start crying and has never fallen asleep without me being there.  Is this bad?  Should I be trying to break this habit?  I am just really confused and have been trying so hard to follow the book because at first it really helped. 

Please share some advise!!!!

It has changed a little in the past few weeks as he started getting up at 7:00, but has since started getting up earlier and earlier. 

Here was our day yesterday- as best I can remember:

Up at 5:45 ate and put back to sleep- then up at 6:50
A: 6:50- 8:15
S: tried at 8:15, but wouldn't fall asleep so fed
(E) at 8:45 then attempted sleep again
slept 9:30-10:00
A: went out and thought he would fall asleep in the car- he did not.
since it was 11:30 and was time to eat again I fed him and then tried to get him to sleep again- another 30 minute nap. 
A: 1:00- 1:30
S: tried to get to sleep for quite awhile- stayed with him the whole time and did wake to sleep- woke up after 45 minutes. 
E: 3:00
S: 3:30 tried for another nap- took until 5:00 to get to sleep- 30 minutes only.
E: 6:00
A: bath and then bed
S: attempted to put to bed at 6:30 did not fall asleep until 7:45 and then woke after an hour.

I know my routine is out of wack and maybe part of the problem is that I stopped writing my days down- I thought we had it down pat as he was getting to know his nap times and would fall asleep if we were out or not.  Not anymore...
He did not sleep well last night and today he woke up at 5:00 and I tried to get him down before leaving for work, but he wouldn't fall asleep.  The sitter said he did not sleep until 9:15!!!  and only for 30 minutes!!  I am freakiing out.  How do I fix this lack of sleep??!!!!!!!!!!!

I am going to attempt to change him to the 4 hour EASY and have decided that I really, really need to write down our pattern.  Unfortunately I am working today and tomorrow (I work 2 days per week) so I will not be able to do this until Friday.  I do have the babysitters writing down what he is doing today and willl do the same tomorrow.  It is just frustrating as I worked sooo hard to get the routine the first time and now I feel I have to do it all over again- the crying, staying in the room for all naps etc.  Should he readjust quicker than when he was just learning????  Any advise on changing from the 3 hour EASY to the 4 hour?
