5 Month Old waking up every 45min-1hr

So my DD slept through the night at two months 10-6am and then down again till 8:30am.  Since then, progressively she has been getting worse and worse and is now up every hour from 9pm till 4am at which point she will eat and then sleep till 7am and up a couple of times till she is up for the day at 8:30am.  Here is our Easy Routine:

8:30am wake up and eat
10:30am nap (she will wake up once after an hour for her paci and fall back asleep for another hour)
12:30am wake up and eat
2:30pm nap (same routine as at 10:30)
4:30 wake up and eat
6:15 short 45min nap
7pm wake up and eat
9pm last bottle and sleep

She then wakes every hour... I give her her paci and she falls back asleep instantly every time until 4am when she refuses the paci and then I know she is hungry and eats her bottle (4oz).  If I try to feed her earlier she will only eat about an oz or two.

She eats 6 times a day around 4oz... she wont eat any more...

Please help... is this a paci problem?  She is swaddled still... is she too old for that?  She has no problem going to sleep, she falls asleep within minutes with her paci and within seconds once I give it to her during night wakings.

Any suggestions would be great!  Thanks :)

PS  I am hoping not to wake her earlier because her brother gets up at 8am and I get him ready for school and then I feed her and we leave the house at 10min to 9am to get my son to school...

It definitely sounds like a paci problem :-\. If she is waking mid nap & will settle for the remainder with the paci & if she's up at night only settling for a paci, then the paci is a prop. I would encourage you to look at the props forum & the pu/pd section to get some ideas of what/how to do this.... that is if you want to wean the paci? Might take you a few weeks of shh/pat or pu/pd (settling in the cot) but its well worth it (in my opinion) & you won't be up all night re-plugging. I had the same issue with #2 & at 4 months I finally had enough & helped her learn how to self soothe. I never thought she would be capable of settling down on her own - but she did!
