what I can't figure is if it's outside of where her mouth is, why did it hurt so much during nursing?  ??? Maybe the damaged area was more than I could see (ie hard to see a bruise on the actual areola?)

It is a bit mysterious. Unless you were feeling pain from previous damage that was made worse by even quite gentle tugging or the letdown reflex or was related internally to what was happening to the ducts. I think it's certainly possible to have a bruise you couldn't 'see' as you suggest.
