Help with 3 month old Increased NW

Our little guy is 3 months and one week old.  He has always been a pretty sensitive baby and not a very good sleeper - from birth has slept between 8 to 11 hours a day including nighttime and naps.  He has actually been a pretty good night sleeper, sleeping 6-8 hours consistently since his second or third week of life, but not napping very much during the day.  Over the last couple of weeks I've managed to get him on a pretty good EASY schedule and his cues have become much easier to read.  His daytime napping has also increased (although I often resort to putting him in the swing or letting him nap on me for his afternoon nap because it seems like the only way to get a good 1.5 to 2 hours out of him). 

Suddenly, over the last four or five days he has begun NW.  Usually around midnight, 2, then 3, then 5 and 6 am.  I have been trying very hard to get him back to sleep in his crib, but the last two nights I ended up letting him sleep on me from 5-6:30 because I am getting so exhausted.  I also have an 18 month old at home so it's difficult to keep things quiet for the baby, who is extremely sensitive to outside noises.  Other info - I am breastfeeding but supplement about 2 ounces of formula with a supplemental nursing system per feeding due to low milk supply (past breast surgery).

Here is his average schedule:

Wakes around 6:30-7am

E - 7 am breastfeeding about 30-40 minutes
A - is up for about 45 minutes to 1 hour before he starts to get tired and fuss.
S - usually from about 9:15 am.  30 minutes to one hour.
E - 10/10:30 am breastfeeding about 30-40 minutes
A - for about 45 minutes to 1 hour.  We often go for a walk at this time and he falls asleep in the baby bjorn.
S - sleeps for 45 minutes to 1.5 hours.
E - 1 pm breastfeeding about 30-40 minutes
A - for about 45 minutes to 1 hour.
S - This is usually his longest nap.  Will sleep up to 2 hours if on me or in swing.
E - 4/4:30 pm breastfeeding about 30-40 minutes
A - 45 minutes.  Will sometimes take a catnap or gets really fussy around 6/6:30pm.  Bedtime routine is a bath, massage and feeding.  Starts around 6:45pm and baby is usually put to bed around 7:30 or 8 pm.
S - 7:30 or 8 pm.  He is getting better at falling asleep in his crib with a tight swaddling and paci and I can usually get him to sleep within about 20 minutes.

Over the last 4 days wakes at midnight, 2 and/or 3, 5 and 6 am.  The first night I fed him when he woke up.  But for the last few days I just soothe him (with a paci, patting and sometimes have to bounce to get him to stop crying) then feed him at 3am, and again at 6 am when he wakes up.

One more note:  The grandparents have been here the last 3 days, but the NW began before they got here.

Thank you so much for your response.  I definitely agree that he gets OT. 

Suddenly over the last three nights, he has completely worked it out...!  Two or three days ago he was getting cranky,I went to put him in his crib and he started to cry, but my 18 month old was in his high chair, so I said "baby, you're just going to have to hold tight in your crib for a sec".  Well, by the time I had wiped my other son's hands, baby was asleep! So the last two days for bedtime and naps I just swaddle and use paci, then set him down to go to sleep.  The first night I went in 4 times I think.  Then last night he feel asleep all by himself (and slept 9 hours!!).  Today for naps he's been pretty successfully going down on his own.  I am amazed!  He is still only sleeping pretty short periods through the day, but this feels like enormous progress in very little time.
