What is this?

My 19.5 mos old has always been a pretty consistent night sleeper and is now waking anywhere from 4:45am (today) to 6am screaming/crying.  We've tried the following:
*laying him down and telling him it is still night-night time (although sometimes he isn't standing but just laying there crying)
*giving him milk or water
*holding him for a minute and puttng him down again
*letting him cry (for about an hour today)
*bringing him in my bed, where he just talked and pulled my hair
 
  He has been doing this about 3-4 times per week for 3 weeks now.  I took him to the ped last Monday to rule out an EI or sickness.  The ped told me it might be reflux so we tried Mylanta for 3 days but I don't think that is it. She said if that is not it then it is sep. anxiety but my LO will easily go to the kids center at the gym, let others hold him, play with me out of sight so I'm not convinced it's that.  Not sure what else to do.

Is he cutting his molars? EW at that age around here were pretty much always teeth. Whats his routine like during then day? Can you post what a normal day looks like?

 At BW we are strongly opposed to CIO and dont discuss it on the open forum. I would advise against it as there can be alot of developmental issues at this age and it could well be SA at play. With DS it always came via sleep depsite being a confident little man otherwise.

His schedule typically looks like this:

Wake 6:45/7am
Nap (PD at noon) 12:15-2:15/2:30pm
BT (PD at 7:30) asleep 7:45/8pm

About 13 hours

He has all 4 molars and took him to the peds last week to rule out EI or sickness, she said he could was totally fine.  No eye teeth can be seen but I'm sure they are on the horizon.  She said to quit the meds since she doesn't see anything moving down anytime soon.  She also said maybe it is reflux and to do Mylanta for 3 days and then reevaluate....didn't seem to be it.  Pediatrician then mentioned SA and said the same thing that it could manifest at night even when they seem asleep like my LO.  She said don't do anything for that, just ride it out!

It could we be SA, routine looks fine to me. I owuld ride it out for a week or so and of no change then perhaps its time to think about capping the nap?

Last night we PD for bed at 7:05pm and he went right to sleep, no EWU.  Hoping he is caught up from the EWUs, what's so puzzling to me is the fact that it only occurs 3-4 times per week.  So often enough to be an issue but not every night to figure out a plan and know what to do.

What I might think about doing is keeping a log of when it happens. It could be cumulative OT or cumulative UT!!

 DS started EW when his nap needed capping, your DS os pretty young for that though.  :-\ What would happen would he'd have a long nap and then normal BT and then wake early from too much daysleep, because he was having EW he then needed the nap and so the cycle continued....

So we had 3 nights with no EWUs and then yesterday he refused a nap....so worried a problem is starting.  He slept the night before from 7:20-7:20am so a great night.  We probably should have PD at 12pm like we have been but it was the weekend and things got pushed to 1:15pm.  He was still playing around until we finally got him at 3pm.  I went in 2 times to lay him down and tell him it was night-night time.  He was having too much fun!  I thought he'd be okay with a longer WT.  Stressing about naps again b/c of yesterday and hoping it was a fluke.  We put him to be at 7pm and he slept till 7 today.
