Teething and not eating!

Can anyone enlightened me on how long teething pain lasts after a tooth has come through the skin.  My son had his first tooth break the skin last Friday and has just been irritable since.  He wants anything and everything in his mouth and is hardly eating.  He does has reflux so he isn't a good eater anyway, but now it is worse.  I have tried ice, teething gel, Tylenol, etc.  Does anyone have any other advice or encouragement.  He is 5 months old.

How is his reflux being treated - if with meds which one, what dose and what does he weigh? It could be that his reflux is flaring up because of the teething which is putting him off his food.

He is on Prilosec 5mg twice a day.  He is 16lbs 10.5oz.  It is a low dose, but was actually just increased by his doctor at my request last week.  He has only spit up once in the last few days suprisingly, but burps a lot.  I know he could still be having silent reflux.  Does teething really cause that much pain for an extended period of time which keeps them from wanting to eat well?  I thought once the tooth broke the skin it was less painful.  I guess he could have others moving up under the skin.  I have checked his mouth, but don't see any new ones emerging yet.

Teething can go on for a while and teeth move around in the gums long before we can see any evidence of them. It's mostly likely the combination of reflux and teething I would think, if it goes on much longer it would be worth speaking to his Dr again to see if he will increase the meds to a decent dose for weight.

Have you tried teething remedies or pain meds just before a feed to see if it makes a difference?

Laura, I have used teething gel, ice, and Tylenol throughout the day.  How much Prilosec do you think he needs to be on?

Also, do babies with reflux just consume less typically?  I know one of the things you look for when starting solids is a baby drinking a lot of milk and still not seeming satisfied.  Well, I don't think my son will ever be at that point.  He consumes so little for his age and weight as it is.  I am just so confused and worried about the amount he takes in everyday.

You will need to speak to the Dr about the dose, but on the "crying over spilt milk" site (it's a NZ site) it gives a low dose of omeprazole (which is what prilosec is) at 5-11.25 mg for 7.5kg weight. I know he's on the 5mg dose twice a day which puts him at the higher end of the low dose range, but if each individual dose isn't high enough he won't be getting any benefit from it. A high dose for the same weight is listed as 11.25 - 22.5mg.

As for the milk intake it is very common for refluxers to take in less but it's only a worry if they aren't gaining weight or meeting milestones. My two were both big babies and took an average of 20-25oz a day - DD in particular stuck like glue to the 91st centile on that intake. Solids can actually help them take more milk in some cases as it helps keep the milk down, they reflux it less so feel more like eating. Solids did make my DS worse, but he wasn't medicated at the time.

How much is he taking in 24 hours, which formula is he on and which flow teat?

Thanks Laura.  He is taking 24oz a day and is between the 50-60th percentile age/weight.  He is meeting all of his milestones actually a little ahead of schedule.  He is on a presciption formula called Elecare.  This was started on this by the allergist for a MPI.  I am using Dr. Brown's bottles with the #2 nipple which is for 3-6month age.

I would try the next level nipple just to see if it increases his intake, just be sure to use it for all feeds for a couple of days to let him adjust :)

Thanks Kara.  I did move up to the next size nipple which he is tolerating well, but he is not taking any more than 24oz a day.  He seems very happy and content though.

I wouldn't worry - if he is happy with that amount, it is right for him :)

Babies are great with self-regulating what they need.
