11 week old - short naps

Hi all,

My LO is 11 weeks (but was 9 days late, so maybe more like a 12 w/o?).  I would categorize her as a Textbook/Angel baby.  For the most part she is very happy and easy going.  She is healthy and doesn't have any issues with reflux or other things.  I have been having some problems with short naps the last few days and I'm not sure what to do. 

Here is our routine:

7am - eat (breast feed)
7:15 - awake - usually we just talk or do some tummy time or reading
8:15 - start our winding down routine by reading a couple of books, swaddle, sing, pacifier
8:30 - she is asleep on her own
Then 30-45 minutes later she is awake.  I wait 5-10 minutes to see if she will settle back down but she doesn't, so from
9:00-9:30 - I calm her down and leave again when she's quiet.  She may be quiet for 15-20 minutes but doesn't go back to sleep and starts to cry again.  I get her up and we hang out until

10:30a - eat (BF) (has trouble staying awake)
11- awake - run some errands and she falls asleep in her car seat
11:30-12:30- sleep and then wakes up on her own
12:30 - 1:30 - we hang out

1:30p - eat (BF)
2:00p - awake and we do tummy time, talk, or she lays on her activity mat
3:00p - book, swaddle, sing, pacifier
3:15p - 3:35p - sleep for 20 minutes and then wakes up.  I let her fuss/cry for a little while to see if she will self-soothe with her fingers.  She doesn't so I go in and calm her and try to have her stay in her crib quietly even if she's not sleeping.  She tolerates it for 10 minutes and then cries again so I get her up and we sit and rock quietly. 

4:15p - eat (but she doesn't seem very hungry and keeps bobbing of the breast)
4:30p-5:15p awake - tummy time, activity mat
5:30p - sleep in baby carrier (APOP on this one because she won't sleep in crib for this nap)

7:00p - eat (BF)
7:30p - awake - bedtime routine - bath (every other day), read books, swaddle/pacifier, songs
8:00p - usually falls asleep on her own with a little fussing

10/10:30p - DF with 3.0 oz formula and then sleeps through the night until 7am

I'm concerned that her nap times are so short during the day, but then she still sleeps through the night, so I'm not sure if it is a problem or not.  She still gets about 14-16 hours of sleep a day which I know is in the typical range of her age.  She gets fussy between 5p-8p but I know that is typical "witching" hours and she still goes to sleep very easily on her own.  Other than that, she doesn't seem to suffer from the short naps.  I just don't want it to be a problem a month from now. 

Is it time to transition to a 3 1/2 hour schedule?  (She is almost 13 pounds).  If so, how do I do that?  If not, how do I extend nap times? Any other suggestions?
