you elevate the mattress from underneath with a blanket or book, like Louise said.
numberonenole - about the blankets - you use receiving blankets, so they are thin. you can use 2, if they are bigger (like carters brand), 3 if they are smaller (gerber). lay the blankets down end to end so the ends overlap a bit. then you can roll them up, making one really long roll that won't come apart. you can then use it as a wedge from shoulder/armpit, down around the feet (or under the bum & have the feet go over the top), to the other armpit.
safety - the blanket shouldn't really be around their face, but if they squirm towards it, receiving blankets are usually thin enough to breathe through. [obviously, though, don't do any of this if you are not comfortable with it.

you can also use that trick to keep them from sliding around in their bouncy seats when they are very little.
