my six month old nightmare

Hi
My baby sleeps very well at night, usually waking between 7-7.30 but is hopeless at sleeping in the day. He used to do at 45mins at a time this now seems to be between 20-30mins and really struggles getting himself off to sleep often lying in his cot for up to one and half hours before dropping off or not going at all, he is constantly tired yawning, rubbing eyes etc and refusing to play without grizzling.
He is also a nightmare to get to bed a night, we have a consistant routine of milk, bath and bed. But he will start screaming as soon as his bath finishes and nothing will calm him down. If you pick him up this makes him cry more. The only thing that seems to work is leaving him to cry for 10-15 mins at a time then returning to do shh-pat which will eventually get him to sleep, can take anything from 40-90mins.
But things are not improving
Any suggestions please

Ed wakes between 7 - 7.30 has milk and breakfast, by 9-9.30 he is very tired and needs a nap. I put him down and he may or may not go to sleep, if he does its for a max of 30mins before waking and still very tired. If left he will not go back to sleep. the same happens at the lunch time nap.
i have tried pu/pd but this seems to make make him cry even more.
At bedtime he has milk at 6.30, bath then bed at 7. After bath I put him in his sleeping bag sit him on my lap and gently sing some songs then put him in his cot. He screams the moment we enter his bedroom and will not calm down even when i'm holding him.
Lokking forward to some advice

Have you tried swaddling just for naps? I know he's old for it, but our 7-month-old won't sleep for longer than 30 mins without swaddling because she's disturbed by her movement. She doesn't need swaddling at night. Maybe worth a try?
