only sleeps one sleep cycle

My name is Shannon and I have a 4.5month old boy Jakson.
He is breastfed 4hrly during the day and has some rice cereal in the evening. He is having rice cereal as his weight was starting to level out and our paediatrician recommended it.
During the day we follow feed/play/sleep routine which has worked really well since birth. Jakson settles himself to sleep usually with not much noise. He wakes after one sleep cycle and will not resettle himself. I have tired and tried to let him self settle to no avail. I go in after a period of time and face him away from me and shh/pat him until he stops crying and then roll him back and he then settles himself back to sleep. If this fails I take him for a walk in his pram and he immediately goes back to sleep.
I have been resettling for 7weeks now. Will I always have to do this or will Jakson learn to do it himself? I am mentally exhausted. My day consists on trying to get him to sleep through the cycle. He stays awake for 2hours so he is really tired before he goes to sleep. He was only staying up for 1hr 30mins and still only slept for 45mins. Someone suggested to me to keep him up for 2hrs and he should sleep the 2hrs. No luck yet.
Any advice would be greatly appreciated.
Also during the night Jakson self settles around 8pm and wakes twice during the night. He isn't fed during the night anymore. I feed him around 7am. When he wakes during the night it only takes about 2mins to shh/pat him and he goes back to sleep. Is this normal? He was sleeping through the night and is now waking. I have tried leaving him to settle himself again but he then screams and takes me ages to get him back to sleep again.
He isn't cold, he is bordering on being too hot so that's not the problem. Any other reasons?

I've never attached a link before so if it doesn't work go to the sleep boards under naps and then you'll see the support thread for 45 minute nappers. 

It seems that there are quite a few babies that do that darn 45 minute thing around 4 months.  My ds was an awesome napper and then the monster started around that age and we've been working on it!  Started off using wake 2 sleep for a good 3 weeks consistently and the past two weeks have been getting progressively better (except for the odd day).  Anyhow, hope to see you on the support thread...

no you wont always have to do it.  he should grow out of it eventually as he learns and grows older.  in the mean time it is so hard on the parents.  we were there with you.  have you tried Wake 2 sleep?  this is somehting that worked for us at times but doesn't always work for everyone.  It is definatly worth a try if you havn't done it yet.

hi, thanks for your replies.
yep we have tried W2S and PU/PD.  Today things are looking up though.  Jakson slept from 7pm-6.55am this morning and i did dreamfeed him at 10.30pm.  i fed him at 7am and kept him awake until 9am.  he went down without a fuss because he was that tired.  it is now 9.55am and he is still asleep!  maybe, just maybe he might sleep longer this morning.  sometimes he does sleep 1hr 20mins.  if he sleeps this long should i get him up and not try and resettle him?  it is hard to resettle him after this much sleep compared to 45mins sleep.
