As far as a sample 6mo routine, every baby is different, especially depending on what types of milk feeds he is getting - is he breast fed or formula fed? Many breast fed babies don't make it to a 4hour routine.
BW recommends that babies be on a 12hr day and 12hr night routine. Six months can be an awkward time for some babies because that is when the transition between 3 naps and 2 naps typically happens. He sounds like he isn't getting nearly enough sleep for a baby his age. My baby is almost 12 months old and is getting on average 14hrs of sleep every 24hr period. It sounds like the most your baby is getting is 12 hrs. That being said, he may be a baby that doesn't need quite as much sleep as some, but I still think there's probably room for improvement for him. 30 minute naps often indicate that a baby is overtired. Typical A times for a 6mo is about 2.5hrs, maybe 3 hours after a really good nap, and on a 4hr routine (i.e. milk feeds every 4hrs).
Can he self soothe? How do you put him to sleep? Do you have a wind down routine? If you formula feed, how many oz's is he typically taking at a feeding?

A 7-9:30 first A times are often the shortest. A solid feed would be given here - this would be where you would introduce new foods so that you'd see the reaction before bedtime, and avoid tummy upset if there was a reaction hopefully it would be through before bed. How long you wait between milk feeds and solid feeds really depends on the baby. My dd needed a good 1.5-2hrs between the two in order to take much solids, but many moms will do solids as soon as an hour after milk - experiment and see what works for your baby!
S 9:30-11 if he is waking early and is grumpy, then you would use a sleep training method like pick up/put down to help him learn to sleep longer. Let me know if you need more info re: this

A 11-1:30 or 2 depending on tired cues. Solids here if you wish. I found at this age dd needed more milk before her nap in order not to wake early hungry, so I did a split bottle -some before this next nap, and finish it up after the pm nap
S 2-4
E 4 finish bottle from earlier if it was a split feeding
A 4-7 solids here if you like
S 7 for the night
You can do a dream feed around 10 if you like - post if you need more info
Many moms are dropping the dream feed in another month or so, so if you don't do a dream feed, I don't know if I'd get all wound up to start one.
You posted that he is a small baby- are there any medical reasons for this? Have the doctors been concerned about it? I'm not saying they should be, just don't want to be telling you something that goes against any medical advice you've gotten
