falling alseep in the crib 9.5 month old

My 9.5 month old Liam is having sleep problems, again!  Before Christmas we were doing great with only two night wakings and he would fall back to sleep quickly.  Now we are struggling again.

Here is our schedule:

6:30 Wakes
7:00 Solids/Bottle 6 oz
9:40 nap 1.5
11:20 Solids/Bottle 5 oz
2:30 nap 1.5
4:00 Bottle 5 oz
4:30 Snack
6:00 Solids
6:15 Bath
6:50 Bottle 7 oz
7:00 Brush Teeth/Bed

The problem is that he is going down fine and sleeping for 2.5 - 3 hours and then wakes for a diaper change and then goes back down for 3 - 5 hours and then wakes every .5 - 1 hour the rest of the night.  The NW start at between 2-3 in the morning.  The past two nights he has ended up in bed with us at 5 and slept until 6:30.  I do not want him to get into a habit of coming into our bed, if he isn't already.

He is just getting over a double ear infection and a virus for which he was hospitalized over night for dehydration. For 10 days over the holidays, while we were on vacation, he was sleeping with us for most of the night because he was sick and wouldn't sleep in his pack and play.

We have gotten in the habit of rocking him to sleep, and I fear we are becoming Liam's prop.

I know Liam needs to fall asleep in his crib, but I don't know how to do it, because if he doesn't want to stay down he'll just crawl out of the covers and pull himself up on the side of the crib and fuss.  I would also like to introduce a lovey, but I am not sure how to do it.  Any advice on getting my little one to sleep better.

Can you explain pd and sa?  I am not familiar with the language.  Thanks for your help.
