Advice on Expressing for all feeds

Hi All,

I've been having issues with weight gain in my DS.  From about wk 12 to wk 17 he put on only 5oz (one week he actually lost weight).  So wk 17 I moved to a four hour routine and started expressing between feeds to top-up.  My LO has always been a super quick feeder (10 mins is a long feed in our house!) so I think as he's getting older he's not patient enough to wait for the milk to be produced and gets easily distracted.  I am thinking I might move to 100% expressing but I have a few questions:

- How much EBM should I give him? He has 5 feeds a day + DF.  When I bottle feed he always takes whatever I give him so not sure how much to give and whether he will self regulate after a while.

- Do I need to express at the same time each day?  How many times a day do you express? And will it get quicker/easier/more milk?  How many times a day will I need to express initially?

- When I woke up this morning both breasts felt full.  He fed on one side only.  I then expressed about 1oz from that side and 4oz from the other side but did not drain it completely.  If I drain both breasts after the feed will there be less milk for him at the 11am feed or will the body just produce more?

- Could I try expressing for some feeds but still BF'ing 7am and 7pm?  (We did this for DD when I went back to work at 6 months.)

I just wanted to ask are you sure he's having weight gaining issues? Are you checking his weight on a home scale or at the doctor's office? At the same time in the day, without a diaper? If it's not the same scale all the time it could account for what looks like a lack of weight gain. Is your doctor concerned with his weight? Check all these things first to make sure there is an issue before stressing yourself out. My 3.5 yr old dd always had weight gaining issues from the time she was born. My first doctor totally stressed me out about it and told me to stop bfing and change to formula! Instead I changed doctors ;D and got a ped that was supportive of bfing. She in fact was not concerned with my dd, she pointed out that both my dh and I are of slim build so it made sense that my dd was in the 10% and less scale. She was not concerned, even the odd time when my dd did not have gains, she pointed out that my dd was alert, happy, and had lots of energy. She also said "well, someone has to be in the bottom of the grid for weight otherwise there would be no grid!".

If you still want to top him up I would not switch to exclusive pumping, it's more work than bfing (from what I've read, I bf my dd for 18 months and never pumped a day in my life). I would maybe just pump the breast that he didn't take and offer that in a bottle the same time (but the day after, so you always have a full bottle from the previous day's pump). I don't think you can actually get them to drink more in a day in total quantity. I think any extra drank at one meal might mean less is taken in at the next. At least, that's how it was with my dd. The only time I was able to somewhat impact her weight gain is when she went on solids as I was able to pick the calories in foods and offer higher calorie foods. While exclusively bf, I couldn't "pick" higher calorie meals.

Hi All,

Thanks for all the advice and honesty.  I knew that pumping would not be easy but didn't realise how much time and effort it would involve.  I have much admiration for anyone who goes this route.  I simply don't think I can do it as I have DD starting school in 3 weeks and my DS does not sleep very well. 

To be honest if it was just the weight gain I wouldn't be too worried but he is up 5 times a night and not sleeping well during the day so basically I am so tired I am beginning to loses the plot.  I had this with DD too but started her on solids at 4.5 months at which point she started sleeping through (guidelines were different 4 years ago).  However, both DH and DD have delicate stomachs so for that reason I would like to hold off on solids for DS.

I decided yesterday to introduce another FF during the day (his DF is FF).  I will try and express at this time and stock pile for a few days then have a couple of days of exclusive bottle feeding so that I can see how much he is taking.  Then I think I'll mix BF/FF/EBM but I'll have a better idea of how much to give him from a bottle and also whether there is any improvements in his night times.

When the Dr first suggested more FF'ing 3 weeks ago I was devastated as I really enjoy the closeness of the BF'ing, however now it's getting to the stage where my sanity, and DS's general happiness (he's definately happier when he is sleeping better) is more important.
