Help! 12 week old becoming a catnaper

 Hi,
I'm needing some help with my LO who has started only napping for 45mins (on the dot).  It all started last week when she began to get really fussy while feeding, i then got mastitis and went on antibiotics.  Her feeding got worse and i used formula to supplement her as i felt she wasnt getting enough breastmilk.  She had been sleeping 2 good 2 hour stretches and another 40 - 1 hour as well during the day, and was sleeping 7pm to 5:30am, with a DF at 10:30pm.  Over the course of this past week she has slept 45 mins for each nap, will sometimes fall back to sleep if we hold her but wont resettle in her cot.  Overnight she now wakes around 3:30 for a half hearted feed and then again at 6:30 to start her day.  The short naps are driving me crazy!  

Any ideas how to get her back into a good sleep pattern?

Today looked like this:

E 6:30
S 7:10
E 8:15
A8:45
S 9:30  woke at 10:15, tried letting her resettle until 10:30 then got her up and was happy in the swing/play gym until E
E 11:15
A11:45
S 12:15, woke at 1:00 again tried resettling until 1:30 then got her up
E 2:15
A 2:40
S 3:30 woke at 4:10, got her up (we had gone to mums to pick up DD1) carried her around until we went home at 5:00
E 5:15
A 5: 35, bath 6:15
E 6:30, she has a bottle at this time every night
S 7:00

its now 9:45 and shes still asleep (as usual) and will have a DF at 10:30, then fingers crossed for the night

she settles herself to sleep at each sleep time, i put her in awake, she grizzles for 5 mins tops and then sleeps so i dont think im missing her sleep window.  just cant seem to get her to resettle after that first sleep cycle during the day.

Any suggestions would be great.

time for an increase in her A time.  at 3 mo she should be doing 1.5 hours A time first thing (right now she's doing only 45 min?).  she is short napping and then her A times are getting too long after a short nap so the day is falling apart on you as she gets ot by the end of the day.  try to get that first A time right and get a good first nap out of her and then aim for 1.5 hours A time after the long nap.
