Moving from BFing to sleep to EASY

Hi, posted this in props with no response wondered if this was where I should have posted:

sorry if this has been explained elsewhere, have looked at the gentle removal plan and not sure it anwers my question.
Little one has always breast fed to sleep, I can take my nipple out of her mouth when she's getting sleepy and if stroke her nose/face she'll drop off.  She will sleep for up to an hour on me but I often can't get her down in her bed - we have a hammock not a crib.  Health visitor has advised me to let her cry to teach her to go to sleep on her own in her bed but I just can't do it. I don't know how to make the very sharp seeming transition from b'fing as a prop to EASY, as sh/pat does not pacify or calm her.  We're stuck in a vicious cycle at the moment of being overtired as won't sleep during the day so I've been caving in just to get her some sleep.

hi there and welcome :)

How old is your LO?  If she is age appropriate, I would recommend PU/PD to wean her from the prop of BF to sleep.  We were in your same boat 4 months ago and my DS learned independent sleep rather quickly (much to my surprise) rather quickly with PU/PD. 

Also, what is the temperament of your LO?  Sometimes knowing this can help in the specific way you do PU/PD.

Definitely agree not to let your LO cry it out - the BW method teaches your LO to sleep without breeching any trust with you and you LO.
