Phew I'm in exactly the same boat here and it's driving me nuts, DS won't sleep for any longer than 45mins and I was managing to extend his morning sleep using W2S and a bit of shh/pat for 2 hrs but his afternoon sleep was o longer than 30mins and they have been getting less and less his afternoon sleep was 16 mins today, I have put Jim down for another rest and  he woke at 16 mins again I tried to pat Jim back to sleep but I didn't work, I just left him in his cot and he cried for about 3-4 mins then I replugged him and he has gone back to sleep. I'm wondering if I'm going in to soon?

My humble and still developing observations:

For us 35-45 minute nap = undertired.  Need to extend A times.  DD is also a crazy loud sleepstage shifter, so I can usually see the jolts coming, but nothing I do to intervene helps much, so I usually just freeze and hold my breath until she's gotten through it.  It's actually best if I'm not there because with WTS and HTTJ, she would see me and smile and want to play and then it was all over.  If she does wake all the way up, I'm just resigned to it, pick her up and go play, usually for another full two hours.

16-20 minute nap, however = OVERtired.  If I can't resettle with shh pat, I just snuggle quietly for a while, and she usually is ready to sleep again within the hour.

Clearly not a perfect system, but it's what's working for us right now.  Sounds like they eventually just grow out of this.

Hi story3girl. I totally agree had ben doing w2s and as soon as my lo sees me he thinks its playtime. if i cover infront of his eyes he just tries to look around my hand. im just hoping that he grows out of it soon. tried a hite noise cd today to see if this helped him progress through 45mins and its exactly 45mins and can hear him stirring. He had 2.5hours of activity time today??

Yup, srs, we're still there too.  All I can say is it sounds like you're doing the right things and hopefully they'll just grow out of it!
