Trying for a routine for 10 weeks.. HELP

Ok, i really need some help now, i thought things would start settling down by now, but they have not.
Noah is 10 weeks and 1 day old.
I think his reflux is under control, so i can not really blame that any more. He does not seem to be in any real pain.
His day time naps are all over the place, I aim to feed him for the day between 6 and 7 am. Due to the fact that he has been so restless from about 4 am onward he seems so overtired after this bottle. So I think the longer I keep him up - aiming for a good hour, the worse he gets. I put him down, after a few dummy plugs and his mobile on, he settles down and has a 45 min nap, after this time, i go in and give him the dummy, reset the mobile, he sleeps for another 10 mins, doing this on and off about 3 times. I get him up, play, put in bouncer etc, until his next feed is due - 3 hourly. Once again he is so tired after this bottle, that I do the exact same thing as above, getting progressively harder to settle as the day continues.

6pm bottle - very tired struggle to get much into him.
6.30pm bath
7pm bed settles instantly, sleeps through his dream feed at 10pm
Wakes approx 2.30am, give him the dummy, next wakes at 3.30am, where I feed him.
Wakes every hour after this for the dummy and it all starts again.

Before you mention the dummy, there are smokers in the household. Obviously just smoke outside.

I have a 4 and 2.5 year old running around which increased the difficulty.

Is being overtired the problem?

Hi hun,
 Sorry you're still struggling. I really think, at 10 weeks, that if they're struggling with sleeping and you're trying really hard, then something may be bothering him. I understand and I believe you when you say his reflux is under control, but do keep in mind that some medications could be dose related and reflux manifests in other ways than the obvious crying after a feed such as unsettling from a nap. *hugs* I'm just empathizing bc I went through months of agony of awful naps and OT accumulation and the end of the day was just bad bad bad.

May I see the rest of your day and how it pans out? You mentioned what you did..every 3 hour feed and repeating the sleep..but it's very helpful to see it all laid out :)

I also found that when I was mapping my days, that when i thought I was doing a certain A time, when I actually wrote it down at the time it was happening it might be way different than I thought. For example, I thought I was doing 3hr routine, but when I jotted down start A time when eyes open and then go back and jot when eyes closed I would have a longer A time by 10min sometimes, and that is a big difference for the little guys.

I too was going to add something about the reflux.  For both of mine 45min nap with 10min resettled nap to follow frequently meant pain from reflux.  Are you elevated and all that good stuff?
