Frequent NWs and I have no idea why, please help

HI,
If anyone can shed some light onto our situation or provide some advice it would be greatly appreciated.

My dd is almost 15 weeks. She used to get up at 4 or 5 to eat and after that she would be up every hour until 7:30-8 when she finally got up, we noticed that when she was eating at that time she was either falling asleep while eating or not really eating a full bottle so we tried not feeding her and just put her back to sleep. When we did that she was still waking at 4 or 5 and then every hour after. Her "waking" is mainly becuase she wants her soother or has broke free from her swaddle, she usually goes right back to sleep, but i still have to get up and go see her. I've tried letting her settle herself she just gets louder and more upset (and i am afriad shell wake up her sister). So my main question is what can i do to help her sleep through the night without waking every hour (or more) after 4 or 5 am??

We have just started the dream feed (again) last night and it didnt seem to help her sleep longer she was still up at 4 and 5 and 530 and 630 and then 830.

her day routine is below (we also have trouble with the 45 min napster but trying to get that working too)

8 -eat
915 - sleep (40-60 mins with any nap, is she wakes early and not time to eat she has activity or might eat early)
11 - eat
12:30 - sleep
2- eat
3:30 - sleep
5 - eat
6:30 - sleep
8 -eat
9 -bed

she used to eat at 9 when she went to bed again a few oz but now we are trying df at 10 -1015

Any advice to help with our night waking or our short naps would be greatly appreciated

Are you trying to extend naps?  How long are the naps now?  Other than the 1st one, I don't see times.

I don't think waking at 5 am and going back to sleep after a feed is unreasonable at this age.  Is she FF or BF?  Her bedtime is so late, that I can't imagine a DF would help.  What time are you trying the DF?

Hi and thanks for responding.

I am trying to extend naps almost every one I can (sometimes we are out doing things with my older daughter and then she just naps here and there in car seat) I try to extend her 2nd and/or 3rd nap while we are home and my other dd is napping too. I didnt have success with W2S but have had some when i go in as soon as she stirs and i put my hand on her  chest and help her get into the next sleep cycle. Currently its working about 50% of the time and i can get another hour, i hope that it helps and it will become more regular. 
We recently have been working on her falling asleep in her crib/playpen by herself. I am now at the point where i hold her until shes groggy and then lay her in sometimes all i have to do is say shhh or sing sometimes i have to put my hand on her chest but she getting it, and a lot quicker at falling asleep. At night all i have to do is lay her in her crib and i sit beside her now and do nothing.

She is FF and eats 5-6 times per day at 5-6oz each feed. Last night she ate at 745 and was asleep in bed at 845 (we are slowly moving her bedtime back - 10-15 mins at a time) Her dream feed was at 1030 and she ate it well but was up every hour from 1 onwards and 3 times between 5 and 6. Usually i can tell her hunger cry but last night it didnt seem like it, she was crying with her eyes closed and as soon as you reswaddled or put soother back in she fell asleep. Do you think she's waking becuase she's hungry?

That DF time is probably interrupting her sleep, since it is so close to bedtime.  Is there a reason you want bedtime to be so late?

No its just the way things happened. Originally (when she was a lot younger) i was worried that I would be up more often or earlier, I was trying to get more sleep in a row because when i put her to bed i would go to bed as well.

What bed time should i aim for, if shes 15 weeks? what time should i expect her to get up in the morning?

It is just a late bedtime for an infant.  A baby's day is 12 hours and most like to be in bed on the early side, especially depending on naps.  Most moms are aiming for a 7-7 routine.  With that type of routine, a DF would be between 10-11.  Did you check out those sample routines?
