LO writhing when feeding

Hi, my 11 week old sometimes writhes when he's feeding. He often rears his head back - taking my nipple with him OW! If he's unswaddled he waves his arms around frantically (and ends up scratching me) and often pops off coughing and gasping or he digs his head in and grunts. It doesn't happen all the time, but most day feeds it does. Any ideas what the problem could be? It's making feeding a pretty miserable time for me.

This reminds me a lot of DS at thst age  :(

Oliver does it when he has wind - I take him off and let him burp then he can feed as normal ...

Wow thanks for the quick replies! I should have said that he has reflux, and has been on ranitidine since he was 6 weeks old. The writhing is newish though, does it mean I need to up his dose? Burping does help a wee bit, I've been giving him infacol to help get those burps out, so will keep doing that.

Do you have a fast letdown?  That's what my babies did when the milk was pouring out at them while they were feeding.

If he has been on the same dose of ranitidine since he was 6 weeks old he may well need an increase - how heavy is he now and what dose is he on?

We've upped his dose a few times, I'm not sure of his exact weight, but it's around 5.5kg. He is on 0.7ml three times per day.
I'm also avoiding all dairy, soy and gluten.
I don't think I have a fast letdown, when I pump, it seems to take ages! How else do you know?

With mine, they would have to swallow really fast and would sometimes choke and cough while nursing.  If they pulled off, they milk would keep flowing all over (once dd2 got squirted in the eye)!

I've been giving him infacol to help get those burps out, so will keep doing that.
for the first few months Oliver needed it for every feed or would bring up lots of milk with a burp, and was harder to burp ....

I'm pretty sure I don't have a fast let down, unless I've missed a feed and then I drip all over the place. There's no squirting though! I'll continue with the infacol and hope that helps.

My DS sometimes does the same thing.  He's now almost 16 weeks and is still doing it off and on.  Some feeds will be perfectly fine and others absolutely miserable.  He started doing it at one particular feed and then one day it turned into to pretty much every day feed.  I haven't figured it out yet actually we're going to the doctor again today!

The behaviour described really sounds like reflux,it is very typical to do all those things, they are indications that are not dealing well with the pain in their oesophagus as they feed.  Think of their oesophagus as red, inflamed and very sensitive and aore, as they are constantly having stomach contents washed upawards again and again and if its a bit acidic, then ouch!!   

Refluxers are grunters by trade, it is a pain noise really.  Coughing and gasping can be fast let down too, but with reflux it often is that fact that they are in pain when feeding. Rearing the back again is often a pain behaviour, again when the throat is hurting a way to counteract it is to arch.

  I can't quite do the math here as I am unsure of the concentration of the ranititinde, but the dose should be about 4mg/kg 2-3 times per day.   Ranitidine is weight sensitive (as you know  :)).

It sounds like the reflux is bothering him, and maybe he needs a dose increase or else another med, or maybe a thickener before each breastfeed.   We used a carob bean gum thickener for our refluxer with great success.   Had to give it before every breastfeed with a little water in a syringe.  It allowed the milk to thicken in the stomach to the consistency of cream, it really helped with pain.

You can also try positioning so that he is slightly more vertical, such as the "rugby hold" where his body goes under your arm and feet behind your back.  This way you can prop him up a bit to help gravity work for you.  I had fast letdown/oversupply with dd2 and it gave her reflux symptoms/choking while feeding.  This position helped, as did me reclining and letting her feed facedown, or side-lying with her head propped up a bit.  You can't always tell about strong letdown with pumping, as your letdown will always be stronger for baby than for the machine.  A bit of elevation helps with "standard" reflux, too.

I just checked our dosage chart and it says 5.5 kg to give 1.4 to 2.8 ml a day, he's getting 2.1 ml over three doses. I wasn't told by my doctor that it's more effective to give 15-30 mins before a feed, so have started doing that and will see how that goes before upping the dose. Anyone done this before? How does it work with the EASY?
