HELP with my nearly 5mth olds NW's

Hiya,
I see there are a few of us in the same boat. Thought Id start my own post instead of hijacking someone elses ;)
Well like others, my bub was doing awesome at night waking 1-2 times max, then around 3mths or so things started going backwards.
It kinda coincided with his day naps becoming better. So he was no longer in that demand feeding stage.
We also had some issues with breastfeeding, nipple thrush, early teething & him becoming distracted.
I am now sooo exhausted & am not convinced he needs two night feeds as he feeds good at one of them but not the others.

So a bad night is him waking 3-4times eg 10-11pm, 1-2am 3.30-4am then awake for the day around 5am.
most nights are 2 NW with an EW.

I've recently found out that I have 'low capacity' with milk supply, which means I dont produce alot of milk between feeds, so apprently more frequent feeds are necessary.
He was feeding 3min each side max, but is now feeding alot better & esp if I feed in his bedroom.

Our days havent been all that consistent as they need to accomodate my 3yo's kindy run 3 days of the week. I am about to implement a routine to accomodate this & do it each day, but unsure of the best way to do this, so any advice/tips on this would be great.

So an eg of our routine is:

5-5.45am wakes, we leave him until 6am to get him up.
6.30am E (breastfed both sides, I offer the breast 3 times each or for 30mins max on advice from lactation consult.)
7 A
7.30-8am S - depends on how long he was awake in bed for before 6am. usually has 1-1.5hrs
8.45-9am- I wake him if still asleep to drop DS1 off to kindy.
9.30 E (both sides)
10 A
11.30 S
1-2pm wakes anytime between these times however I aim to wake him around 1.30 so he can have a catnap around 3.30/4pm
2.30ish E (both sides)
2.45 A
3.30-4pm S (hit & miss here, usually takes it if he's had over 2hrs A time)
4.30-5pm E (both sides, I offer now as its the last BF I do & I need to do dinner)
if no catnap then:
5pm 1/2 feed 5.15ish bath 5.30ish rest of feed & into bed 6pm
if catnap then the times are pushed out about 30-45mins
we are yet to try a DF, thats starting tonight.
some nights wake 11ish takes awhile to actually cry out, usually chatters. DH offers a bottle & he takes less then 100mls
then might wake around 2amish, I'll feed & he'll take both sides but can wake again around 4ish, I offer one side & then will be awake again around 5.30ish.
Lastnight he woke 1.30am I gave both sides, was back in bed 2am but woke 4.30am, hubby offered 100ml bottle but gave him one go on it, had about 80ml & back to sleep in 15min, he woke at 5.50am chatting but went back to sleep at 6am until 6.50.

We've tried offering water at the 1st NW but it hasnt worked with extending the sleep.
He is swaddled, doesnt have any props to go to sleep, has always self settled quite well. he has had some teething issues & a cough, but dont know if thats causing the NW's?
Most of the times he wakes (& has been like this from birth) he starts out cooing & chatting away then eventually cries out, sometimes after 30min of being awake.
he seems ready for solids & have offered him a bit, but know that its early days & not likely to make him sleep through.

the day given isnt the same day every day. but will be the aim. when I can accept waking him from every nap :(

Not sure if anyone can see whats going on?? Im happy with 1 night waking, until he's on solids properly ;)
hoping someone might know what to do to help us :)
