First off I want to say thanks to Zacsmumme for getting us this far - you are wonderful  :-*

Secondly, sorry it's a long post!

DS will be 1yr old in a couple of weeks, has always tended to be a little above his age on A times.
We've had a proper mess up since he was in hospital in November but now I'm thinking this isn't just about getting back on track but more likely I need a new strategy.

First A of the day has always been the longest, I back tracked to 4hrs to get back on track with the first nap whilst he was still poorly and recovering from his stay in hospital, then 4.5hrs.  Been getting 1hr nap there and if I extend immediately (wait in his room for WU, PU cuddle tell him to go back to sleep approx 1 min to relax into my arms PD, firm hand or rub with key phrase) I can get another 30min to an hour.
Today tried 4hr 45 first A and he saw himself through the hour mark up to 1.5 hrs and couldn't be extended.

Second nap isn't working at all.
He's refusing it and if I work really hard to get him down he doesn't wake up.  It's like he's gone into his night sleep and I have to work hard at waking him up gently trying not to upset him (and haven't been able to wake until 1.5 or 2hrs).  This results in very late BT.
If he totally refuses the second nap I try to go for EBT but after trying for the nap for a while it ends up impossible to fit in dinner bath and bed without getting a really long A.
If we are out I can AP this nap in the car on the way home to get 40 min or 1.5hrs, seems pretty random how long he decides to sleep in the car.  This is far from ideal, I can't take him out every day to AP this nap.

Lots of NWs in the early part of his sleep regardless of having his second nap or not.  I'm not sure what this is.  He was waking in the night a LOT following the hospital stay (both prior to midnight and then again around 3 or 4am) and it's taken a while for the NWs and feeds to abate.  What remains now feels like sleep problems rather than illness/recovery, but he has gas then too so wind pain is as likely a culprit as sleep problems.
Until recently always had a 10 to 11 hr night, never 12hrs.  Recently has been quite random, any NWs seem to make WU later as though he is making up for the time he was awake, even had one 14hr night but I think that was with a 2 hr NW.

example of a pm nap refusal
WU 6.30
A 4.5hr
S 11.00 - 1.00 (extended at the 1hr mark)
A try for 4hr (but resulting in 5.5hr or 6hr A)
S refusal at 5pm, try until 5.30pm or even 5.45
EBT really difficult to fit in dinner and BT routine so ends up down at maybe 6.30pm (5.5hr A)

today:
WU 5.45am (earlier than I expected)
A 4hr 45
S 10.30 - 12.00
A 4hr 05
S 4.05 - 4.45 in car
A 2hr 30
BT 7.15pm

My thoughts are to go for a longer first A, hope for a longer nap (try to extend) and have a long A to bed.
Something like:
WU 6.30
A 5hr
S 11.30 - 1.30
A 5 or 6 hrs
BT 6.30 to 7.30pm

Any ideas or thoughts?

Have you tried the 1 nap day?  Stick to an 11-12 hr day when you do a 1 nap day and see how he gets on.  It's not unheard of for a kid to be on 1 nap at his age...mine was :)

Yes I've tried the one nap.  He gets very tired around 4pm when he used to have a set nap but we get through it with cuddles and a bit of chill time and once he's past the body clock thing of wanting a nap then he copes reasonably well up to BT.
Yesterday his one nap wasn't extended although I tried for a good while (he needed to poop!) and he was SO tired later that I offered him a second nap but I suspected he wouldn't take it, he just spent 20 mins or so in his cot chatting, mantra, then moaning and finally called for me to get him up.

Last 2 days have been
A 5hrs
S 2hr 15 (extended at 40 min, again at 40 min)
A 5hr 25
night S 11hr 30

A 4hr 55
S 1hr 15 (disturbed by poop)
A 5hr 55
night S 11hr 45

Does this look about right so long as I try to extend the nap?
I feared his early evening disturbance would be worse with OT but it seems to have improved a little.
