Night Wakings driving me mad!

My DD will be 1 in less than 2 weeks.  I am still breastfeeding and she in my opinion is getting too much of her milk intake at night!  She only feeds a couple times a day (or so).  She does eat solids 3x a day and snacks, but she is hit or miss with the quantity (loves berries and all fruits tho:)
Her schedule is as follows- roughly

wakes usually around 615 - 7
breastfeed
plays
breakfast around 8
nap around 9-930 sleeps for 45m-1hr15
plays (try to give her a snack of cherrios or puffs also)
lunch around 1230 - 1
sleep (always keep her up for at least 3-3 1/2 hours after she wakes from am. nap)again anywhere from 45 min - 1hr and half (have tried W2S and PU/PD when waking too early)
plays
supper by 6 (husband home around then try to wait for him but sometimes she can't manage)
bath, followed by wandering after her sister while we prep for bedtime
bedtime routine - feed, book or lullaby (sometimes she is too cranky for book - sing her a song and put her to bed)
Lately for bedtime she is ticked if I leave the room at bedtime but will let me leave at nap - I say _ Mommy be right back and she goes off on her own (daytime) at night I try that and she screams - if I go in there and just stand by the crib and whenever she gets up I say bedtime and pat on the mattress, she lays back down and I momentarily put my hand on her back - but I stay by the crib and she keeps checking to make sure I am, then goes to sleep on her own.
Now as for NW - she always wakes between 11 and 12 - I feed her and put back in crib, she goes back to sleep, then she's up again around 3 or 4 and 5 or 6 - no night is ever the same, but there have been very few where I have gotten just two wakes.  She has slept once for 6hours straight and once for 7 hours straight somewhere in there - and some nights with 5 hour stretches, but this the life I have become accustomed to.  I go back to work in two weeks and I'm starting to just go in at night and not even looking at the clock and feeding her almost everytime - I am a zombie.
any advice would be appreciated

Also I just thought about how I do try to wait at least a few minutes before going in to see if she will settle herself.  She just gets herself so worked up.  (She has quite the lil temper)
More history is that she did sleep with me and was fed back to sleep up until 3-4 months ago, when I read the baby Whisperer books and used PU/PD to get her in her crib to sleep and to stop using the boob as a prop.  It feels like I am falling prey to that again tho:<
