Going from breast milk to formula??

My DD is 5 months and I am stopping breastfeeding gradually over the next few weeks. She now has 2 bottles a day but only takes 1-2oz (formula) at each feed. She goes 4 hours without a feed after but surely this isn't enough?

I worry about under or over feeding. Anyone been in similar situation?

I've linked this post to your post in Bottle Feeding - Going from breast milk to formula?? as hopefully you will be able to get better guidance about the amount of formula to offer without over/under feeding.
