The routine looks great to be honest - I wouldn't have thought that was the issue....
Is she unwell or teething at the moment? Frequent night wakings can be related to discomfort. Was she waking crying or happy?
The other thing that springs to mind is that you are rocking her to sleep at the start of the night, and the frequent wakings may actually be due to that - in that when she comes into light sleep and briefly wakes (as we all do, though we don't remember it), she doesn't know how to get back to sleep without your help. The fact that she settled so quickly with a bit of rocking does suggest that it may have turned into a prop for her - what do you think?
Teaching her to sleep independently (if that is something you want to do, of course) will help with prop-related wakings, though if there is another issue like discomfort at play then LO will wake regardless.
