Hi everyone,
DS is 5 months 1 week, and now entirely formula fed for last month or so.  He has always been difficult to feed hence I gave up on the bf early this time.  He is really still sooooo difficult to feed.  At night he does much better, after 6pm he will take 4-6 oz and for the df and nf the same, in fact he wakes for a feed 4 hrly on the dot lately. 
By day its completely different.  This morning he wouldnt even let the bottle come anywhere near him, and his last feed was 02.30 so its not that he is full up.  Commonly for his first feed he may eventually take 0-2 oz over the space of1-2hrs.  For his other day feeds he will commonly take up to 3oz then stop, you literally cant get him to take any more, he just fights it cries turns his head etc.
We have previously tried gaviscon when he was a few weeks old but no difference.  Also I have used comfort formulas but as they are thicker and you need a faster teat he's not happy with that and comes out too slow through a medium teat so he gets frustrated.  We are using MAM 2 teats, the 3 is really much faster, and C+G first milk.
I dont think its a distraction issue I've tried feeding in a quiet room etc same thing.  Also I dont think its that hes just filling up at night he is probably stil having less than his requirements at night. Any ideas gratefully received thanks Becky x

Could you just list out what times you are feeding and how much he is taking?

It does sound to me that he is taking in most calories needed overnight.

Ok starting last night
2240 df 5oz
0230 4.5 oz
0730 woke up wouldn't feed at all over space of 2 hours
1200 2.5 oz
1600 2.5 oz
1830 5 oz

Have you started solids at all?  How much does he currently weigh (momma's best guess is fine!).

He is a fair bit below what I would expect for a 5 month old for sure...

Just wondering if he might be teething?  Does he get mad about eating during the day or is he just not interested at all?

Averages between 15 to 28 on a good day average about 20oz in 24 hrs, I did start solids but on advice here we stopped them about a week ago but no improvement tbh. I think he may be teething but I have thought this for about two months still no sign! He's 15lb 7 had him weighed on thurs just on 25 th centile

What centile was he at birth hun??  Is he dropping off his curve?

Does he have reflux?  If so, is he medicated?

He was born 25th went up to 50th then back down we tried gaviscon when he was a few weeks but it didn'takw a difference

I know you've tried Gaviscon unsuccessfully in the past, but this sounds EXACTLY like my daughter before we got her acid reflux under control.  Her would eat 2ish ounces without a problem (just enough to activate the acid production) and then refuse to eat more, cry if we brought the bottle near her, etc.  She'd eat in her sleep, however, as she was more relaxed and the acid didn't bother her.  Her weight gain dropped off dramatically.  She was on Zantac at the time, and her pediatrician changed her to Prilosec with amazing results - it's supposed to take several weeks to work, but by the end of the first day she was eating better, and within a week, she was at normal levels.  You might consider talking to your child's doctor more about the reflux possibility.

Hi all after thinking about this some more and him getting more and more unhappy during and after feeds I realised this has been getting worse since he has been completely formula fed, since beginning of december which got me thinking maybe its related to cows milk allergy and that may also be causing reflux? Anyway I took him to GP this morning who agrees it could be, as he also has eczema that is getting worse and has prescribed me a trial of hypoallergenic formula nutramigen 1 to see how that goes and will let her know next week if there is any improvement if not try something else so fingers crossed!

Sounds good hun... give it two weeks for sure (dairy will take that long to come out of his system).. but if it is MPI, you should see improvement within a couple of days.

If you don't see much change, you might be best to ask for Neocate :)

arrggghhh started with the new formula 24 hrs ago, today he has been worse than ever and now only taking 30-40 min naps, he has done this x 3 today and now have a v long way til bed time

ok he has been on nutramigen 3 days now, still not feeding any more, still distressed I am actually noticing him bringing milk back up now, and today I notice he now has eczema behind his knees, he only ever had it on his face before how can this be?  Think I am going back to Drs in the morning or am I being inpatient?

He might need an elemental formula hun... Neocate over there in the UK.

You should see some level of improvement fairly quickly but if he is getting worse and not better, I would go back to the dr too.

Hi I went back with him on monday and GP only wants us to try one treatment at a time so we can see whats effective, as I was convinced his reflux was the problem and formula not made much difference we decided to try ranitidine and normal formula.  She prescribed 0.7ml tds and I started him on sma staydown which gave him terrible constipation he doesnt normally get to the point of blood in stool (fresh and small amout) so I switched back to normal cow and gate.  I dont think the ranitidine is really helping and have just spoken to gp who suggests trying aptamil pepti before we try neocate and going to check his weight this afternoon

Eczema makes me think it could be milk allergy, not reflux.  Reflux can be caused by a milk allergy and some babies can come off meds with an elemental formula...

Re: the Zantac - how much does he weigh now?  Just to confirm, his dose is 0.7 mls twice per day?

Hi no sorry he is on 0.7mls three times a day, I'm a bit unclear on whether I'm meant to keep giving him that actually but I have been.
I got him weighed yesterday afternoon and he has actually lost weight, down 3oz from 2 weeks ago the poor boy.
I am actually surprised that he lost weight after a while you think that you're making up his irritability and everything else as every time the dr sees him 'he seems happy enough'.  I try to explain that I'm not saying he never smiles you can distract him for short periods, its just his default setting is miserable at the moment.  So in an awful way its like I have proof something isnt right yk?

Given that he is now losing weight, I would suggest asking for a referral to a pediatrician.

I do think this is more than just a crabby pants as you do, something is going on whether it be milk allergy/intolerance or reflux (or both)...

As for the Nutramigen/Aptamil Pepti debate - they are the same thing essentially... Though the Pepti is supposed to taste better...  The next step would be Nutramigen AA or Neocate.
