Just seeing this.
I had a thread about the BT bottle about a year ago. I was brushing DS's teeth before the bottle. I was concerned about the milk on his teeth all night and also didn't want to ruin our wind down routine. From the posts on my thread regarding danger to teeth I decided I had to change our routine. it scared me. Some LOs have tooth decay before age 2

What I will say though is that often these changes to routines are far more scary for us mummies than they are bothersome to our LOs. I was astounded by how easily my boy adapted and accepted the new routine. I had felt sure his wind down would be destroyed and he wouldn't sleep. Not the case at all. He barely batted an eye at the change.
Yes many people do continue the bottle, that is your personal choice, but I would also encourage you to alter your routine to include teeth brushing after the bottle and use another method of soothing for sleep. IMHO the milk for sleep habit will continue for as long as you allow it, so maybe ask yourself if you'd be happy for this to be happening at age 5 or 6 and your LOs teeth still not being brushed before sleep?
There are a variety of options to alter your routine, but one possibility would be to offer the milk prior to brushing teeth, brush well, then offer a water bottle for soothing prior to sleep (you could make this change over the course of a week: full bottle before teeth brushing, small milk bottle offered after teeth brushing, plus a second bottle on hand with just water in it for if she is requesting more prior to sleep, reduce the small bottle of milk down and down each night until you are only offering water). I know you said your LO won't take water but if she wants the bottle then she'll adapt pretty quickly.
Also if she is refusing to have her teeth brushed at any time of day this (IMHO) really does need tackling.
