I have an almost 13 month old son who has a very hard time falling asleep and staying asleep. We still co-sleep - he sleeps in our bed. For a while, a few months ago, he slept in his crib at least for the first part of the night, but after some cold and teething episodes he ended up sleeping with us all night and it stayed that way. This is a multi-problem issue

1. The falling asleep part. Dylan struggles to fall asleep. He rolls and moves around on the bed for a good 30 - 60 minutes before he is finaly able to fall asleep.
2. Waking up frequently. Our LO still wakes up every few hours.
3. Reasons for waking up. Unfortunately, he still requires bottle feedings throughout the night. As soon as he sucks down the milk in a bottle he falls back to sleep.
4. Problem #3 is caused by his poor eating habits throughout the day - not very interested in solid foods, mostly still wants to drink his milk. Doesn't eat enough during the day and compensates for it at night. Also, LO is a snacker and gets his milk in small portions (about 4 to 5 oz at a time) many times a day and night.
I realize that lot of this is our fault. However, I also realize that this is a high time to do something about it and change his feeding/sleeping habits, but I don't even know where to start. I would like him to be able to sleep in his crib and ideally transition to his own bedroom. I would like to help him to fall asleep and stay asleep easier as well as to eat more during the day so that he is not hungry at night.
If any of you have similar experience or any ideas on how to tackle this please let me know. I appreciate all feedback.
