Forceful Letdown, Frothy poops and pumping

Hi there,

I have had trouble breastfeeding since about day 1. My 4.5 month old son has always been fussy at the breast and has never really had good poops. Lately, he has a lot of diarrhea and frothy poops (sometimes green). I read that it was because of a forceful letdown or an imbalance of foremilk/hindmilk. So, in a desperate cry, I decided to pump and bottle feed him. I have sometimes supplemented with formula and he has been digesting the formula so well. I have also already cut dairy (and gave him lactose free formula). I'm just not sure where to go from here. I want to keep giving him breast milk but am finding it hard. Last night he woke up 4 times because of diarrhea and gassiness.

Do you feed from one or both sides during a bf? With DD2 I did single side and ended up block feeding due to fore/hindmilk imbalance.

I tried to feed from just one breast but when my baby sees that the milk slows down, he gets angry. So, I decided to pump so that he would get all the rich milk but it doesn't seem to be helping. Maybe I should throw out the 1st ounce on each breast when I pump?

Have you tried breast compressions? Breast compression

Sorry I have no idea on the pumping, was never very successful with it myself!

Have you considered a milk protein intolerance (as opposed to being lactose intolerant)? Does my LO have food intolerances?

Did you cut dairy out completely including all the hidden stuff in things like deli meat, crisps, sauces?

Hi everyone,

Thanks all for your help. His poops were getting worse and worse (even saw some blood) so doc suggested to give him exclusively formula for the next couple of days. That has helped. Will now try back breast milk with a very limited diet and go from there.

Oh dear.
If you want help ensuring you are cutting out all dairy or whatever is the suspected problem then these FAQs might help. Ingredients to avoid for food allergies - Milk, Soy, Eggs, Peanuts, Gluten

I completely cut dairy with C and noticed a huge difference. He also had neocate formula but that was for emergencies :) ask away if you have any questions...

It is my understanding that if you have green, frothy poop it is due to foremilk/hindmilk imbalance and hyperlactation.  I would try block feeding for a bit.
