Help to stop NWs

DS is 5 months tomorrow. Here is our EASY
Wu often around 6 am but v variable
E 6.30/7 but never very hungry BF
A 2 hours regardless of WU time
S around 8.30/9 for 30 mins on the dot then PUPD or shushing to resettle. WU around 10
A waiting for E
E 10.30 formula, followed by solids about 1 hr later
A
S 12 for 30 mins then resettling until 1 1/2 hrs or if out picking up DD may only have 30 mins
A
E 2.30 pm BF
A
S around 3 or 4 depending on previous nap. A time always 2hours. Again will wake at 30 mins
E 5 pm solids and some milk BF
A bath 6.15-6.30 with DD
E breast or bottle in room often falls asleep even if sat upright!!
S 7-7.30 depending on length of feed.

Multiple NWs, first at 10.30ish, fed as if DF. Then 1-2 hours after back to sleep for rest of night. Feed only if > 4 hours since last feed so only once or twice more. Trying not to feed after 3am to keep hungry for morning. Variable wake up times, usually feeds for at least 20 mins if fed. Never seems hungry during the day. Tired signs at 2 hours A time or less!
Resettle at night with PUPD.
Wakes from EVERY sleep crying
Tummy sleeper due to reflux.

Really need to improve the nights as back to work soon full time as a surgeon!!! Weaning BF slowly, loves bottle and formula.

Is reflux under control? Is he on any meds? Still spitting? If reflux is not under control that can explain a lot of what's happening.

Her A times seems pretty age appropriate so the only two things (other than pain) that can be happening is that either she is slightly OT and that's why she wakes after 30min (which usually indicates OT) in that case I would shorten the A times by like 10min to see if it helps. The other thing is that she might not yet manage to go through sleep cycles by herself and for that you can try and use HTTJ (hold through the jolts), there is some info about it here: How do I address habitual wakings?
