my 11 m-o son has a temperature (38 celsius) and is very sleepy. He is on his second nap (first one was 40 min from 11-11:45) and at 3 he went down and is still sleeping. I know sleep is good for a baby, but if I let him sleep, he might not sleep at night, which  might make his fever worse...

I would let him sleep, his body needs it. I would get him up before bedtime to get him changed and see if he wants some fluids and give meds if needs be but then get him back to bed.
