How can I tell when I should use a medium flow nipple?
My son is super fidgety when eating, and I have to burp him several times during a feed, as he's gassy and a big puker....I want to try him on a medium nipple to see if the fidgeting is due to him not getting his food fast enough, but I am not sure...he's four months now, any suggestions???

You can't loose anything by trying a faster teat on his bottle so give it a try. I changed to Number 2 teats when Will was about 3 and a half months as it took him an age to feed and like your LO he was fidgety.
He looked a bit surprised at first and I had to just smile and reassure him that it was ok and he took to them really well and feeding is much better and quicker and Will really seems to enjoy it.
Have a go - see what happens - you can always go back to slower teats if he doesnt get on with the new ones.

My lo was exactly the same - he 4.5 months old and is already on the fast flow teats - size 4 (Advent).

He went through a phase where he was constantly fidgeting and I changed him to the size 3 and after a couple of months this was not good enough and had to change to the size 4.  He loves to guzzle his milk down.  My only problem now is that he seems to be fidgeting again (been on size 4 for about a month now), as the milk does not seem to be coming quick enough - but there are no bigger teats than the size 4.

Not sure what to do now.
