4mth old refusing both breast and formula feeds

Hi,

I am new to this board and hoping for some advice, help for my 4mth old baby. I am finding it very difficult to get him to drink his milk. He will take mostly bottles of either expressed breast or formula. He had very bad reflux so on advice from DR we put him on thickened formula and I add thickener to the breast feeds. The reflux has almost disappeared however the problem is for past 6wks he has had no routine with his feeds. So obviously its not the reflux.I have tried 4hrly and even 5hrly but he refuses to take any drink until he is absolutely starving. It takes many attempts to get him to feed but once he does he drinks fine and is much cheerier for it including me. Otherwise he just cries when I put the bottle to his mouth and I have this grumpy little bub. If I give him water he will drink this with no hesitations. I have tried letting him have little sips of water then changing to bottle of milk but he is too smart. I know its not the formula as he will not take straight from breast most times either. I am so confused as unable to recognise anymore whether he is hungry or just tired. I am tossing so much formula as the thickened stuff needs to be fed straight away. He will take 4 feeds in 24hr period and about 600ml total if that. The only time he will drink well is on waking during night which is now only once. Where do I go from here?

Hmmm...that is a tough one..??  Have you tried different bottles/nipples?  Do you follow EASY?  If so, I would just feed every 4 hrs (or 4.5-5 if that works better).  If following EASY it should be easier to read his cues.  If you want, post what a typical day looks like routine wise...maybe something will stick out at us.  :)

We took Max to doctor last Friday after he refused milk for 14hrs and he has gained no weight in 3wks. Doctor still thinks this is related to reflux re; heartburn not healed thus he has no appetite and possibly relates milk to pain. We have started him on Losec. He always drinks his milk through night so now I am waking him on 4th hr to get the calories in. But he no longer cries for drinks at night anymore like he used to. If I didnt get up he would have a grizzle for 30seconds and fall back asleep. Before he used to cry until fed. Anyway he still fights the bottle during the day. I decided to get all bottles,teats, 2 different formulas and my breast milk with and without thickener. Waited until 5th hour then lined them all up and tried each very patiently. He took about 70ml reluctantly as usual, put water bottle to his mouth and sucked away. I know its not the teats because he takes bottle so well at night when sleepy. If he takes a good feed I will mix what he hasnt taken with cereal and he loves taking that from spoon. My husband even fed him milk from spoon the other day when he refused bottle and he was grabbing at it like he was starving. So hard and messy to do it that way. Today I have had a little more success. He fights for a bit but I use my stern voice instead of my soothing voice and finally he gives in and has taken a lot more than usual. I am offering feed every 4 hrs. He schedule works - wake, play for about half an hour, feed, play, he will have been up total 2 1/2 hrs, then sleeps about 45minutes. I would offer him feed first from waking but the time for feed always falls about an hour short and I cant get him to sleep any longer. He always goes down for his naps well. - Thanks

Just a thought--have you considered a milk allergy?  We had some fussing and back arching (breast and bottle) before a change in diet.

Oh, gosh, your little one sounds identical to my kiddo when he was younger.  It sounds like it definitely is the reflux.  When we switched to a PPI (DS was on Prevacid, but Losec is pretty much the same), things greatly improved.  We also sleep-fed him at night, and would try to feed by spoon or syringe. 

If the Losec does not seem to help with the feedings, maybe you can ask for a referral to a GI specialist.  They may be able to rule out allergy, or get you a referral for a feeding specialist.  In the case of my soon, we did get the reflux under control, but he had a bottle aversion for months afterwards. 

Are his stools normal looking?  Sorry if this is TMI,  :-\  but no mucous or blood?
