12 month old still at 3 bottles

Bella just turned 1 year and I am still giving her 3 bottles a day, this with her 3 meals a day.  Mostly i haven't weaned her because she has had a head cold and the teething is really bad right now.  How do i go about doing this?  just start cutting out one bottle per day, and do i replace that with anything?  this is a child who dosen't eat much either so i try to make it up with milk if i can.  she won't take milk out of a sippy but will water and juice.  I really don't think she will notice but right now she gets a bottle before her morning nap after breakfast, a bottle before her afternoon nap after lunch and an evening bottle right before we brush her teeth and then straight to bed.  I know the evening bottle could take awile to transition.  But how do i go about doing this and what kind of food schedule should she be on?  3 solids and 2 snacks?  thanks for advice and help:D

When Sophia was one year old (by the way happy birthday!!) I stopped giving her the third bottle and all she did was to slowly increase the amount of solids on her own.
I think most children "know" how many calories they need and they adjust to changes.Her feeding shedule at that time was:
8.00 Milk
10.00 Fresh fruit
1.00 Lunch
6.00 Dinner
8.00 Milk
Sophia was also never much of an eater and the only thing that has changed today is that she eats something for breakfast (bread with honey, cereals or porridge) and
at around 4.00 she gets a snack. Usually she want bananas or cookies.
I used to be worried about her weight because she was very skinny but my pediatrician told me that as long as she behaves normal and does not look tired, I should not worry.

gah, this is me exactly, ds is 11.5mo.

i don't know what to do about the bottles, either.  we have been working on putting the milk in a sippy, that's been going well.  i think our next move is to move the milk closer to the meals.
