night wake after 90 minutes, takes 2 hours to put back down

My 4 and a half month old has recently been waking screaming about an hour or an hour and a half after putting him to sleep in the evenigs. I go in to find him arching his back and trying to climb up the wall of the bed. Then, it takes about 2 hours to put him back down to sleep. I can't figure out why, and how to stop it. The cry sounds like a mixture of pain and fear, and he continues to cry with his eyes closed for about 15 minutes after picking him up. He seems to stop crying when his eyes open, so I wonder if he's crying in his sleep.

I don't think he's very hungry as he doesn't nurse well during these wakings.
He is quite active, kicky, and wiggly when he wakes up - for 2 hours.
He does drool a lot, but the pediatrician looked in his mouth and thought he was not teething.
He is learning to crawl.
Should I pick him up whe he cries like this? or shush-pat him still in bed?
any other ideas? any suggestions would be appreciated!
