how to get 15 month old off bottles

Hi,

My ds will be 15 months on Saturday.  He still has 2 8oz bottles a day, as well as a 4 oz bottle with each nap.  (My mom watches him 4 days a week-she's responsible for the nap/bottle thing...over time I let it go since he sleeps through the night and that's my main priority.)  Anyway, he drinks fine from a cup, has since about 8 months, but only if it's water or juice.  He is still on formula rather than milk, as he is having lactose difficulties.  When I give him a cup with formula in it, he will take a sip or two at most.  How can I get him off the bottle when he won't drink the formula in a cup?
