Difficulties settling to sleep at night

My DS is very difficult to get to sleep at night and I need some ideas on what to do.  Basically our day goes like this:

7.30am - I wake him up and breastfeed him
8.30 - 9am - into bed - goes to sleep pretty quickly now that he can suck his thumb
9.30 - 10am - wakes spontaneously.  Breastfeed.
11 - 11.30am - into bed - a bit harder to settle him but not too bad
12pm - wakes, takes some time to be resettled.  Usually sleeps for 20 mins and then wakes again but eventually will settle into a deep sleep
2 - 2.30pm - I wake him up and breastfeed him
3.30 - 4pm - into bed - can take a while to settle him to sleep at this time (esp with older sister demanding attention!)
5pm - wakes spontaneously.  Breastfeed. 
5.45 - 6pm - bath, breastfeed and into bed (by 6.30 - 7pm) - usually falls asleep quickly BUT always wakes after an hour and then takes up to two hours to be resettled back to sleep!  Sometimes I try feeding him again thinking that he may not have had enough but that doesn't help (he'll fall asleep feeding only to wake again within 20 minutes).  We've tried getting him up for a short play but that doesn't help either!
3 - 5am - wakes for a quick feed then back to sleep till 7.30am

So what can I do to get him to settle properly at night and not keep waking after an hour?

how old is your dd? What is short play? Play time? I'm sorry, just trying to understand what that is...

Hi

He's 10 weeks this week.  When I said "short play" I just meant getting him up for some awake time.  We thought that he might not have been properly tired.  Didn't work though - it just delayed the settling problem.  We normally do the shush/pat but its just not working at this time of night.  If he does drop off to sleep its only for 20 minutes or so and then he wakes crying again and we start over.

Maybe lo is overtired or overstimulated. Your LO found his fingers fast!!! Well since he will be three months soon, why not try and put him on the three hour easy?
