Nap problems leading to sleep problems?

My 12 week old has never been a good independent napper.  I started trying shh/pat several weeks ago with varying success.  Sometimes he would fall asleep only to wake again in 30 minutes.  Sometimes he will fall asleep and then wake up furiously sucking his fists, as if hungry, even though he only nursed 1-1.5 hours ago.

At this point, he is sleeping very little during the day and every nap time has turned into a battle, with him screaming and me pleading with him to fall asleep.  It seems that not only are the nap problems continuing, now they're affecting his nighttime sleep. He's been awake now for 4 hours, and won't fall asleep, regardless of what we do (pat/shh, rock, walk around, nurse, etc).  Help!  He used to be a great night sleeping, falling asleep around 8 pm and waking around 6 am with one night time feed.  Any ideas? I'm at my wit's end.

Sounds like he is overtired. Can you post your approx usual daily routine so we can see what we can tweak to help you?

Ok, here was his schedule today (I held him for his naps, trying to get him caught up on sleep).

5:30 am - wake
5:30-6 - nurse
6-6:45 - awake
6:45-8:45 - nap
8:45 - 9:15 - nurse
9:15 - 10:30 - awake and bath
10:30 - 12 - nap
12 - 12:30 - nurse
12:30 - 2 pm - awake
2-2:30 - nurse (after trying to put him down for a nap at 1:30)
2:30-4 pm - nap
4-4:30 - nurse
4:30-5 pm - awake
5 pm - 7 pm - nap
7-7:30 - nurse
7:30-8:15 - awake
8:15 - 2 am (sleep - hopefully)
2-2:30 nurse
2:30 - 5:30 - sleep

Today could be a bit off since he's been so tired.  We haven't had much of a schedule since I started trying to get him to nap in his crib.  Since he'll only sleep for 30 minutes or so in the crib, how do I make up the rest of the nap time he needs?
