What formula should you try if possible MSPI?

Hi -

I am posting for a friend.  Her ds was born on May 8th and is a very unhappy, fussy little guy.  She has taken him to the chiropractor and that has helped.  But he is very fussy overall and seems like he is very uncomfortable.  My friend had him on a milk-based formula, then soy, and then Nutramigen (sp?) for only a few days.  She said he seemed a bit better on the Nutramigen but then was fussy again.  However, I think she had him on it for only 2 days...  In the US, which formulas are recommended for MSPI babies? 

He is not arching his back or spitting up, so I don't believe it is reflux.  He is writhing around a lot and drawing his knees up to his chest and crying.  Looked like tummy pain to me.  He just seems like such an unhappy little guy and I feel bad for my friend.  She has a 21 month old who has started putting her fingers in her ears and screaming when her little brother starts screaming!  :'(  So any advice would be appreciated.  Thanks!

I prefer Alimentum to Nutramigen and in particular the RTF version. But you need to give any formula at least 2 weeks before making a change/decision (unless you see extreme symptoms like projectile vomiting, blood in stool, diarrhea, etc). If that doesn't work Neocate is an option.

Your description could definitely still be reflux. Not all reflux involves spitting up or arching - some lower esophageal pain almost looks like gas pains. I'd suggest a visit to a Pediatric GI specialist.

My 6.5 month old has been on Isomil formula (soy) since about 4 months (she also had reflux and hated drinking her bottles), but about two weeks ago, she started refusing her bottles.  At that time, I switched to 3/4 Good Start and 1/4 Isomil and she took it fine.  Last weekend, I omitted the Isomil all together and she started having serious gas problems.  She is waking almost every hour at night and when she does sleep, she whimpers in her sleep.  She is also teething, but Tylenol doesn't seem to help her sleep at all, so I think it must be the gas or her reflux is back (her ped took her off of meds when we switched her to soy because things were getting better).

I'm thinking that it is either an MPI issue, so I tried to give her Alimentum, but she wouldn't take it.  I've heard that Nutrimegan tastes worse than Alimentum, so I haven't tried that.  Any suggestions for getting her to take one of these formulas?
