I don't know if this is happening to you or not since your entire routine isn't posted, but I found around this age, maybe a bit earlier, dd was waking around 5am for a bottle, and then went back to sleep for 2 hours or so, and then woke at 7. She wasn't hungry for another bottle at 7, so there was A time before she was hungry, and by the time she got hungry, it was right before a nap, so then the whole rest of the day was thrown off. If she took an earlier night feed and woke hungry, it wasn't nearly as hard to keep the order of EASY IN the right order
If that is what you are experiencing, hopefully it will get better with time as she will hopefully start sleeping later before needing a feed-do you do a dream feed? That really helped us get dd through the night without the early feeding.
If that is not what is causing your EASY troubles, I don't have a lot of advice, other than making sure there is at least a smidgen of A time between feeding and sleep so you don't get into the bad habit of feeding to sleep will help a ton.
One other thought - 45min naps can often signal that your baby is undertired. It can also be developmental, but I am all about testing A times to see if it might be undertired. My dd was an AWFUL 45min napper at that age. When we moved to a 4hr EASY at 4mo she instantly started taking 2hr naps, (which your lo is not quite old enough for yet). Of course, hindsight is 20/20, and I really think I wasn't letting her have enough A time. She is at the age that a full term baby would possibly be moving to a 3.5hr EASY. So, all this to say, you may want to tinker with her A times a little. Most recommend that you do this very gradually - extend by 10-15min every 3 days or so. The last thing you want to do is force A time and cause your baby to be overtired and only take 30min naps, so gradual is best.
