Night Wakings - goes to sleep by herself

My almost 2 year old has several night wakings each night.  She goes to sleep on her own and when she wakes in the night we typically do WI/WO once and she goes back to sleep but then wakes up again in a couple of hours.  Some nights we do WI/WO several times then she goes back to sleep.  This has been going on for almost 6 months now. 

I think she has become dependant on WI/WO - is that possible?  What do we do?  There may also be some separation anxiety going on - what do we do about that and how do I know it is that?

Her routine is as follows:
6:30-7am - wake
12-12:30 - nap (1.5-2hrs)
7-7:30pm - bedtime

We had a very similar situation and what we did is wean from wi/wo to speaking.  Basically, I said "sleepy time, Lillian.  Time to sleep" every time I did wi/wo and then I didn't walk in, I just said it from the doorway and this worked for us.  Could there be something in her room that is waking her up or scaring her?
